A social worker wears many hats as they help hospice patients, and their families cope with the emotional and practical challenges that surround end-of-life care. Their work is rooted in empathy, support, advocating for the rights of the patient, ensuring they are treated with dignity and respect, and striving to make patients feel heard and empowered to make decisions about their care.
“Social workers are a cornerstone of the care we provide, offering compassion, support, and expertise to patients and staff alike,” said Angelic Health Director of Clinical Services Marie Porreca, RN. “Their contributions help create an environment where everyone, whether receiving care or providing it, feels valued, understood, and empowered. A steward of patient wishes.
The social worker is above all else the advocate for what the patient wants. From going on one last fishing trip, to seeing a newborn grandchild, or even a reunion with an estranged friend or family member, social workers help to turn wishes into reality. This includes assisting with funeral arrangements. Does the patient want a special song played, or have specifics on where they would like to be buried? The social worker develops a life review and asks the questions that others may not to create checklist of their desires as they face their final days.
A counselor and advocate.
The social worker assesses the patient’s mental and emotional status and develops a plan to address anxiety and depression, or cultural traditions surrounding dying. This review includes identifying and working to resolve family dynamics, past trauma and other issues that often arise at the end-of-life.
A guide through the process.
The social worker is a link to securing resources needed to care for the patient, including filling out documents, naming the power of attorney, and decisions on resuscitation and life-sustaining measures. They also provide funeral home or cremation resources and, when appropriate, works with the Veterans Administration for a military burial; or assist those who want to donate their remains for scientific study.
A vital link between patients, their families, and the healthcare system. They understand that a terminal diagnosis affects more than just the body. “It impacts the mind, emotions, and social dynamics,” explained Porreca. Through their counseling, crisis intervention, and resource coordination, they help patients navigate complex and often overwhelming aspects of their care. Whether it's assisting with discharge planning, connecting families with community resources, or offering emotional support, our social workers are there to ensure that patients receive the comprehensive care they need for recovery.
A keen observer.
From noticing and addressing safety issues in the home such as loose cords, rugs and other possible hazards, the social worker also notices other important things to the patient. Family photos tell a story that often words do not. This helps the social workers to better know their patients and enhance their care plan to provide the most meaning and benefit to the patient.
Supporter of the family and care team
Once the patient has passed, the social worker, as well as others from the care team, are there to support the family in their grief, offering guidance, and a shoulder to lean on, or to cry on.
